SS-31
Elamipretide (Mitochondria-Targeted Tetrapeptide)
SS-31 (elamipretide) is a synthetic tetrapeptide that selectively concentrates in the inner mitochondrial membrane, where it interacts with cardiolipin — a phospholipid essential for electron transport chain function. It is the most advanced mitochondrial-targeted peptide in clinical development, with published data from multiple Phase 2 and Phase 3 trials. Researchers study SS-31 because it addresses mitochondrial dysfunction at the structural level, not just the signaling level.

Mechanism
SS-31 (D-Arg-Dmt-Lys-Phe-NH2, also known as elamipretide/MTP-131/Bendavia) is a synthetic tetrapeptide that concentrates >1,000-fold in mitochondria due to electrostatic attraction between its positively charged arginine/lysine residues and negatively charged cardiolipin in the inner mitochondrial membrane (IMM). It stabilizes cardiolipin-dependent cristae structure, enhances electron transport chain (ETC) efficiency (particularly Complex III and IV), improves cytochrome c electron-carrying activity while reducing its peroxidase activity, scavenges reactive oxygen species, increases ATP production, and inhibits mitochondrial permeability transition pore (mPTP) opening. This prevents calcium overload and cytochrome c release (anti-apoptotic).
Evidence note
Emerging — Active clinical trials (Barth syndrome Phase 2/3, heart failure, kidney disease). Published in top journals (PNAS, Nature, eLife). Mechanism is well-characterized. Not yet FDA-approved but multiple clinical programs running. More human data than most peptides on this list (excluding approved ones).

FAQ
How does it differ from MOTS-c?
Both target mitochondria but through different mechanisms. SS-31 stabilizes the electron transport chain directly, while MOTS-c activates AMPK and metabolic pathways.
Is SS-31 in clinical trials?
Yes. As elamipretide, it is in clinical trials for Barth syndrome and other mitochondrial diseases. It has FDA Rare Pediatric Disease and Breakthrough Therapy designations.
Why do some users experience initial fatigue with SS-31?
The initial fatigue/malaise effect is well-documented in the community and typically lasts 1-2 weeks before resolving. It may relate to mitochondrial remodeling as dysfunctional mitochondria are cleared. Community recommends methyl donor supplementation (B12, betaine, glycine) and starting at lower doses (2mg/day).
